Accident Synopsis
The aircraft G-BOWO experienced a landing gear malfunction during its approach to Wolverhampton Airfield. The left main landing gear did not fully deploy, leading to a hard landing on grass. Despite attempts to manually lower the gear and a low fly-past for visual confirmation, the gear remained inoperative, resulting in damage to the aircraft.
System Description
The Cessna R-182RG features a hydraulically actuated, retractable tricycle landing gear system. Each landing gear leg is actuated by a hydraulic actuator powered by an electric hydraulic power pack. The system includes a manual hydraulic pump for emergency gear extension and a squat switch to prevent inadvertent retraction.
Metallurgical Examination
A metallurgical examination of the detached pivot assembly revealed a fracture in the splined shaft area, attributed to cyclic fatigue during normal operation. The examination indicated that the failure was not due to external damage but rather a design inadequacy that could lead to similar failures in the future.
Recent Maintenance History
Prior to the accident, the aircraft had reported hydraulic fluid loss from the right brake, but no issues were identified with the landing gear actuators. There s a hstory of smlar falures on the R82 and the 72RG whch share a smlar, but not dentcal, desgn of the pvot assembly to that on G-BOWO. History of the flight The arcraft was approachng Wolverhampton followng an uneventful flight when the pilot selected the landing gear down. However, he dd not receve a green ‘down and locked’ ndcaton. He re-cycled the landng gear several tmes, but to no aval. He then performed a low fly‑past of the ATC tower and was informed that the left man landng gear was not fully down. He attempted to lower the landng gear usng the manual hydraulc pump and attempted ‘energetc’ manoeuvres n roll and ptch n an attempt to dslodge the gear, all wthout success. Havng brefed the passengers, the plot carred out a landng on the grass Runway 0; the arcraft contnued n a straght lne untl the left wng began to drop and the arcraft then departed to the left, comng to rest on the sde of the adjacent tarmac Runway 0. 93 © Crown copyrght 2008 AAIB Bulletin: 8/2008 G-BOWO EW/C2007/10/03 System description The arcraft has a hydraulcally actuated, retractable, trcycle landng gear system; each landng gear leg s ndvdually actuated by a hydraulc actuator suppled by an electrcally operated hydraulc power pack. When the landng gear s selected DOWN, hydraulc pressure causes a rotary actuator to operate a pvot assembly va a splned shaft, and each man landng gear strut rotates forward and outboard (see Fgure ). Once the landng gear has locked down, mcroswtches for each gear leg trgger a respectve green lght n the cockpt, and the gear selector returns to the neutral poston. An emergency hand pump s avalable for emergency extenson of the gear. A nose landng gear squat swtch prevents nadvertent retracton whenever the nose gear strut s compressed. Recent maintenance history of the aircraft There had been a reported loss of hydraulic fluid from the rght brake for some weeks pror to the accdent, but no cause had been identified. Following recovery of the arcraft after the accdent, the actuator assembly was removed, whch revealed the surroundng area to be awash with brake fluid. There was no hydraulic leak from the landng gear actuators. In September 2006 G-BOWO experenced a hard landng on the nose landng gear n gusty wnd condtons, whch was reported nAAIB Bulletn 2/2006, file ref EW/G2006/09/02. Damage was reported to be lmted to the collapse of nose landng gear and damage to the propeller. No nspecton was carred out on the man landng gear pvot assembly. The pvot assembly had separated n the area of the splned shaft. The fracture surface had been partally damaged by smearng durng rotatonal movement, most lkely caused durng the repeated attempts to operate the gear pror to the landng. A number of fatgue fracture ntaton stes were found at the edge of the fracture face, adjacent to a lubrcatng hole (see Fgure 2). The examnaton concluded that the separaton of the pvot assembly had resulted from a cyclc fatgue mechansm durng normal functonng of the landng gear n servce. Previous safety action A Servce Bulletn (SB), reference SEB90- and enttled ‘Main Landing Gear Pivot Inspection’, was publshed n 990. It s applcable to the Cessna 72 and 82 models wth retractable landng gear, for whch the pvot assembly desgn s smlar, however G-BOWO, Seral Number R820046, was amongst a number of R82 arcraft not affected by the SB. The SB requred nspecton of the man landng gear pvot for cracks n the splne area; replacement pvots were avalable whch were desgned wth an mproved fatgue lfe. The SB also states that: ‘this inspection must be repeated any time an airplane has experienced a landing gear overload condition or if the brakes have a “spongy” operation that cannot be attributed to brake component wear or improper servicing.’ An accdent occurred n 99 to a Cessna Model 72RG, N9592B whch made an ntentonal wheels-up landng at DuPage Arport, West Chcago, Illnos followng repeated unsuccessful attempts to lower the landng gear. Metallurgcal examnaton by the Natonal Transportaton Safety Board (NTSB) showed that: ‘the splined aluminium shaft on the right main landing gear pivot assembly had failed in torsional overload. Cracks were observed at the roots of many of the spline teeth and were observed to have propagated a significant distance into the shaft.’ The NTSB found that there have been a significant

number of Service Difficulty Reports (SDRs) on the Cessna Model 72RG whch ndcated two falure modes related to the crackng or fracture of the pvot assembly shaft. They were, firstly, the loss of braking action or brake fluid due to cracks in the pivot assembly shaft, and secondly, mechancal separaton of the pvot assembly due to the falure of the splne shaft. At that tme on the R82, none of the cracks were related to accdents or ncdents and there were no reports of falure of the man landng gear due to the fracture of the splned shaft. The vew of the NTSB then was that the pvot assembly for the 72RG: ‘is not adequate for long term service and that the design on the new pivot assembly’s splined shaft should be changed to improve its structural integrity.’ Three further SDRs were rased on R82 arcraft, n 996, 200 and 2002; all had cracked pvot assembles and brake fluid leaks which were found during mantenance. The NTSB ssued two safety recommendatons to the Federal Avaton Admnstraton (FAA) n 993. They recommended that the FAA ssue an Arworthness Drectve (AD) to mandate Cessna SB SEB90- on Cessna Model 72RG arcraft wth man landng gear pvot assembles whch have been n servce for 2,000 hours to more, or whch have been subjected to excessve sde loads or other hard landng condtons (A-93-74). Secondly, NTSB recommended that the FAA should requre Cessna Arcraft Company to change the desgn of the splned pvot shaft n order to mprove ts structural ntegrty and durablty (A-93-75). The FAA responded to the two recommendatons n 995 havng completed an nvestgaton nto these falures. They conducted numerous cyclc tests usng the orgnal pvot assembly forgng and the new mproved forgng; they reported no falures n 900,000 cycles. The FAA dd publsh a General Avaton Arworthness Alert n Advsory Crcular 43-6 whch remnded plots of the mportance of reportng hard landng or other severe condtons, so that proper nspectons could be carred out. The NTSB vew was that there stll a need for inspection of current pivot assemblies and classified the response as ‘closed – unacceptable action’. In May 200, the FAA ssued an AD (200-06-06) whch mandated Cessna Servce Bulletn SEB90- at Revson 3 on the 72RG, but not on the R82. Revision 3 introduced a service kit modification for the pvot assembly for the 72RG. For the R82, Revson 3 only requred the removal of a bushng to facltate the nspecton f ths had not been removed durng an earler nspecton; f t had been removed, complance wth Revson 3 was not requred. Due to mnor desgn dfferences, Revson 3 s not effectve for all R82 arcraft and G-BOWO, Seral Number R820046, was one of those arcraft not affected. Discussion The metallurgcal examnaton showed that separaton of the pvot assembly had resulted from a cyclc fatgue mechansm durng apparently normal functonng of the landng gear n servce. Fracture of the splned shaft resulted n the separaton of the pvot assembly and the gear strut from the hydraulc actuator, makng mechancal extenson of the landng gear mpossble. The lmted hstory of falures on the R82 mght support the same concluson on the R82 as the NTSB had made on the 72RG, e that the desgn s not adequate for long term servce; however the work done by Cessna and the FAA faled to reproduce the falure mode. Also, the n-servce hstory of G-BOWO rases questons about the exact cause of the falure n ths case. Nonetheless, the work done by Cessna and the FAA, as well as ths accdent, show that t s mportant for owners and operators to be aware of damage whch can result from operaton outsde the normal operatng envelope.
