No. IASA 22-01 Date: June 10, 2022 Page 1of 1 Subject: Esmet 3000 Pound Capacity Cargo Swivel Area of Concern: Self Locking Hook Roll Pin Movement Distribution: All Fire and Aviation Operations Discussion : Recently, an Exclusive Use Helitack crew was conducting an inspection of their equipment and discovered an Esmet Inc, Cargo 3000 lb. capacity swivel, which had been on their support vehicle over the winter, had the “roll pin” which holds the locking gate in its position protruding out approximately 3/4 of an inch. (Figure 1). They were able to push the pin into it’s proper position and then out of position by hand ( SAFECOM 22-0316 ).
NWCG Standards for Fire Equipment Storage and Refurbishing, PMS 448 , addresses specific fire equipment items used within the National Fire Equipment System. For Self Locking Hooks, 3000 lb.
capacity, it states: Ensure lock latch pin is secure and flush with the latch. If latch is damaged, does not operate as required, is missing hardware, or does not meet dimensional requirements remove from service.

Action Required: In accordance with NWCG Standards for Helicopter Operations , PMS 510 , prior to operations all equipment should be inspected. Any equipment that does not comply with the inspection criteria shall be removed from service.
