ALERT SERVICE BULLETIN 222-26-115 5 August 2026 MODEL AFFECTED: 222 SUBJECT: TAIL ROTOR PITCH HORN ASSEMBLY P/N 222- 012-774-109, REMOVAL FROM SERVICE. HELICOPTERS AFFECTED: Serial numbers 47006 through 47089 COMPLIANCE: Before next flight after the release of this bulletin. DESCRIPTION: Bell was informed of an in-flight failure of a tail rotor pitch horn that resulted in a loss of tail rotor effectiveness and a subsequent loss of control. As a result, Bell is mandating, through this bulletin, the removal of tail rotor pitch horn assemblies part number 222- 012-774-109 before the next flight on the model 222 helicopter variant. The 222 variant experiences the highest loads on the tail rotor pitch horn assembly compared to the 222B and 222U variants. TB 222-94-142 introduced the 222-012-774-109 tail rotor pitch horn assembly as an optional upgrade on the 222/222B and is now superseded by this bulletin. This bulletin also reverts back to the installation of the 222-012-705-105/-107 tail rotor pitch horn assemblies in lieu of part number 222-012-774-109. This is an interim measure to address the immediate safety concern until a newly designed tail rotor pitch horn is made available. Applicability of this bulletin to any spare part shall be determined prior to its installation on an affected helicopter. Prepare the helicopter for maintenance. 2. Verify the part numbers of the tail rotor pitch horn assemblies. -NOTE- The part number of the pitch horn assembly is vibro-etched on the component. The physical differences between the 222- 012-705-XXX pitch horn assembly and the 222-012-774-XXX pitch horn assembly are shown in Figure 1. a. Report the following information for each pitch horn assembly to Product Support Engineering (productsupport@bellflight.com); (1) In the email subject line enter: “ASB 222-26-115; Tail Rotor Pitch Horn Assembly Survey”. (2) Pitch horn assembly part number and serial number. (3) Total Time Since New (TTSN) of the pitch horn assembly. b. If the part numbers are 222-012-705-105 or 222-012-705-107, go to step 9. c. If the part numbers are 222-012-774-109, go to step 3. -NOTE- The pitch link assembly P/N 230-312-001-101 which is installed with the pitch horn assembly 222-102-774-109 is not compatible with the pitch horn assembly’s P/N 222-012-705- 105/-107, therefore the tail rotor pitch link assembly P/N 222- 012-765-101 or -103 must be reinstalled. ASB 222-26-115 Page 4 of 5 Approved for public release. 3. Remove the tail rotor pitch links P/N 230-312-001-101 (BHT-222/222B-MM-1, Chapter 65). Discard bolts P/N 20-057-5-21D. 4. Remove the tail rotor pitch horn assemblies P/N 222-012-774-109 (BHT-222-CR&O- 3, Chapter 64). Discard bolts P/N 20-057-5-30D. 5. Install the tail rotor pitch horn assemblies P/N 222-012-705-105 or 222-012-705-107 (BHT-222-CR&O-3, Chapter 64) using P/N 20-057-5-32D bolts. 6. Install the tail rotor pitch links P/N 222-012-765-101 or 222-012-765-103 (BHT- 222/222B-MM-1, Chapter 65) using P/N 20-057-5-28D bolts. 7. Move directional controls through full travel and check for clearance. 8. Dynamically balance the tail rotor hub and blade assembly (BHT-222/222B-MM-1, Chapter 65). 9.
